The new platform is designed to connect what happens during a patient visit with what happens between visits. Using Ambient AI, PhyxUp can turn a clinical conversation into an EMR-ready note, a patient-friendly visit summary, and a personalized home care program. That information can flow into PhyxUp’s RTM workflow, where providers can monitor patient progress, manage follow-up care, document RTM services, and support billing and reimbursement.
